115508 2023-09-07 Phone Call Care Call Client call made to Ms. Cox, she answered her phone and stated she missed previous call from me due to phone issues. Ms. Cox stated she was doing well. I asked Ms. Cox have her issue with bugs in her apartment been resolved she stated she believe they have. Ms. Cox stated she has no other concerns at this time.
115508 2023-08-18 Phone Call Care Call Client call to Ms. Cox to follow-up on bug issue in her apartment. Received no answer, left a message on her voicemail. Attempt twice with office to speak with property manager to follow-up but both times office was closed.
115508 2023-08-10 In Person Visit Care Call On August 8, 2023 I made a Care Call to Ms. Cox in regard to the bug issue she is having in her apartment. Ms. Cox stated that she received a call from the property manager at Rolling Hills advising her that someone will be out to spray the apartment and she will be able to come back to her home. I advise Ms. Cox I will follow-up with her regarding the completion of this matter. On August 9, 2023, I spoke with the property manager at Rolling Hills regarding the bug issue in client Ms. Cox apartment. Ms. P stated she had someone to spray the apartment, but she will have to do another round of spraying due to the client wasn't satisfied with the outcome. I will follow-up until the matter is resolved.
115508 2023-08-03 Phone Call Counseling I spoke with Ms. Cynthia Cox on August 2, 2023. I called Ms. Cox to introduce myself and Neighborhood Christian Center. I advised her we are here to support her in different areas of need and to help reach her goals. Ms. Cox stated at this time her concern is she has been having a serious matter with bugs inside her apartment. Ms. Cox states roaches are falling from the ceiling and crawling from holes in her walls. Ms. Cox is frustrated that she hasn't received any help after she reported this issue to the property manager. Ms. Cox states she made a report in the months of June and July. Ms. Cox is a single parent with four children. Ms. Cox states that one of her children is autistic and receive Speech Therapy. Due to the bug issue she has been moving around a lot and her child hasn't had his routine therapy. Ms. Cox states she wants to be able to live in her home and get settled with her kids. Ms. Cox states she is not working because she wants to be able to care for her youngest child who is autistic. Ms. Cox currently does not have any transportation and depend on family for help. I advised Ms. Cox I will follow-up with the property manager at Rolling Hills concerning her bug issue. Once Ms. Cox bug issue is addressed I will follow-up with her. I attempted to go by the office at Rolling Hills due to the parking lot being blocked off for pavement I called and spoke with Ms. Patrice. I addressed the client matter in detail with. Ms. Patrice. She stated she was not aware of the issue and she would ask maintenance if they knew about it if not she will have someone to go over to Ms. Cox apartment to check on the bug issue, and also give Ms. Cox a phone call. I will follow-up regarding the matter.
